Organization requesting funds: Contact Name and Phone Number: Mailing Address: Total funding request:  One-time funding request NOTE: Whether funds are approved in whole or in part, a full accounting for the project will be required as a condition of approval 1) Project Name 2) Overview of Project (for this item and following matters, attach additional pages as required). 3) Specify Project Category Type (circle one) Category One: new builds, Category Two: extending facility life, Category Three: reducing net cost of operations. 4) Include a detailed business plan that supports the request. (Business Plan content must include details referenced in the attached business plan content guide) 5) Confirm level of funding generated by the local community as a percent of project costs and dollar amount and provide details regarding the source of funds committed. 6) Attach the most recent three years of financial statements for the organization. If full statements are not available, provide reasons and disclose what limited information is available. 7) Include a listing of contact names and information for the applicant organization executive board members. Include the length of service in their current capacity and number of years on the Board. 8) Indicate the level urgency for this project, (check one)  (emergency)  (urgent),  (important - short term priority / three year window),  (optional -part of a long term plan to enhance the facility and the financial position) 9) Ensure that the information submitted provides the level of detail required for applicant reviewers to complete all portions of a project funding matrix as part of the application evaluation process. 10) In the event that the full amount of County funds requested are not approved or available, provide details regarding alternative plans that the organization has considered for proceeding with the project. 11) Please provide any other information that will assist funding approvers in making their decision. This information is being collected under the authority of the Municipal Government Act and will be used for determining funding allocations by Council. Required Project Business Plan Content Summarize the project scope and include verifiable cost estimates. Provide details regarding funding for the project. For that portion of funds generated by the local community, provide details of firm funding commitments that have been made, and / or accurate supportable projections for fund raising efforts yet to be carried out. Details should include any “in kind” contributions being provided. Provide details that confirm there is a broad level of local public support for the project, especially when additional funding or volunteer commitments are key for the project to proceed. Estimate the useful life of the facility before and after the project is complete. If the project involves an existing facility, include three years of historical facility usage stats. Include a five-year budget of projected operating costs and revenues from all sources. Include estimates of available service hours, and projected usage (number of hours and events) throughout the five-year period covered in the facility budget. Provide details regarding the basis for facility use charges and the organization’s philosophy towards recovering operating costs. Provide supporting details for the population service area and projected estimates for the percent of the local population that will utilize the facility, as well as estimates for the number of users from outside the immediate service area. Include details of potential uses for the facility and the estimated number of annual hours over a five- year period for each proposed use. Provide details regarding the distance to the nearest similar facility that would fill the void if the project does not proceed. Include differences or shortcomings which the alternative facility may have when it comes to filling the identified need for your proposed project. If the project is an upgrade of an existing facility, provide estimates of the current facility life and additional facility life after project completion. For projects that involve improvements for existing facilities, provide estimates of projected reductions in facility operating costs, and or estimates for additional revenues that may be received after the project is complete. Where possible, provide details of historical changes in the local population and future population projections for the service area.
